Agents wishing to secure premium quotation for clients with operations or employees in other states must execute a CompSource Agency Agreement and follow the contracting requirements.
Contracting Requirements
- Agent must hold a non-resident’s agent license in the state(s) which he/she is requesting a premium quotation/coverage.
- Agent will be required to execute a general agent/broker agreement with Midlands Management Corporation.* Midlands requires the following information to contract with an agency:
- Signed general agent/broker agreement
- Agency profile form
- Copy of license for resident state
- Copy of non-resident license in states PMA Insurance Group is quoting/writing business
- Copy of current Errors and Omissions Declaration Page
- W-9
